Get A Copy Of A Florida State Cna Certificate

Duplicate CNA licenses can be requested by mail or obtained online.


A Certified Nurse Assistant in Florida is required to be licensed by the Florida Department of Health. Medical Quality Assurance Services of the Florida Department of Health manages CNA licenses and provides online access to apply, renew and request duplicate licenses. Chapter 464 Part II of the 2009 Florida Statutes governs the rules and regulations of Florida CNAs. Duplicate licenses can be obtained online at the Medical Quality Assurance website or by mail. Allow two to three weeks to obtain a duplicate license if requested by mail.

Instructions Online


1. Obtain login information from your CNA license. Login information was mailed with your original license. If you do not have your login information, Select "Get Login Help?" at the bottom of the page.


2. Select "Certified Nursing Assistant" in profession box.


3. Fill in "User ID" and "Password."


4. Sign in using secure server to obtain a duplicate copy of your CNA license.


5. Pay $25 fee.


Instructions by Mail


6. Send a letter requesting a duplicate license. Include your full name, profession, CNA license number, date of birth and the last four digits of your social security number in your letter.


7. Include $25 money order or cashier's check made payable to the Department of Health.


8. Send letter and fee to: Department of Health, Post Office Box 6320, Tallahassee, Florida 32314-6320.
